Prescribed Application Form:

Application Fee:
A non-refundable application fee of PKR 2,500 must be submitted for each post.
Submission:
Completed application forms, along with all necessary documents (including attested copies of educational certificates, PM&DC certificates, and experience letters), must be submitted by hand or by courier to the Registrar Office of LUMHS.
