📋 Eligibility Criteria (The Undertaking)

Before filling out the form, you must meet the specific criteria outlined in the declaration. By signing, you officially verify that:

  1. Protected Consumer Status: You are a protected electricity consumer with an average monthly consumption of 0 to 100 units.

  2. First-Time Beneficiary: You have never previously received a Solar Home System from any government or non-governmental organization, project, or program.

  3. Asset Maintenance & Security: You will safely operate and maintain the solar system, keep it in your personal custody, and will not sell or misuse it (doing so can result in legal action).

  4. Verification Access: You will grant full access to installation and monitoring teams to visit your premises for physical verification.

The form is straightforward but must be filled out accurately to avoid disqualification.

Section 2: Address & Location Details

Because this scheme targets specific administrative boundaries within Sindh, you must provide your exact location:

  • House No. / Street: Your home address.

  • Village / Town: The name of your village, neighborhood, or town sector.

  • UC (Union Council): Your local Union Council name or number.

  • Taluka: Your sub-district administrative division.

  • District: Your main district (e.g., Karachi, Hyderabad, Sukkur, Larkana, etc.).

  • Division: Your provincial division.

Section 3: Applicant Signature

  • Signature/Right Thumb Impression: Sign your name or place your right thumbprint in ink.

  • Name & CNIC No.: Re-enter your name and identity card number directly below your signature.

🛑 Critical Step: Verification & Attestation

Your application will not be accepted without official verification. The bottom right section of the form must be completed, signed, and stamped by one of the following authorized individuals:

  • Your local UC Chairman or Local Representative.

  • A Gazetted Officer (BPS-17 or above).
