Verification by School Head: The Head Master or Principal of the school must individually sign and verify the academic status for every child listed.
Certification by the Employer: The Factory Manager must sign and stamp this section, officially declaring the applicant's service tenure and employment history at their establishment.
Certification by SESSI/EOBI: An authorized officer from SESSI or EOBI must verify and sign off that the worker is a secured industrial laborer in their records.
Page 3: Bank Account Details & Necessary Instructions

Required Data fields: Fill in the complete details of the establishment, worker’s name, father's name, SESSI/BMC card number, EOBI card number, and complete bank details including Account Title, Bank Name, Branch Code, and full IBAN number.
